R4-25-103 NEW LICENSING REQUIREMENT BY ARIZONA LEGISLATURE All Arizona therapists are now required to have current DPS Level-One fingerprint clearance card for legal practice. License renewals cannot be processed without DPS-issued card. Do not send your original clearance card to the Board . - Initial Arizona Massage License - Plan AheadRememberFirst-Time License Processing takes 3 months Massage Student? New- No Waiting. For expedited licensing, submit your initial application, fee payment and inked Federal print card to State Board & apply for State license up to 120 days before graduation. (Mblex testing mandatory for selecting training not nationally-accredited). (1) The State of Arizona requires a completed initial license application form, submitted online at Board website. Applicant provides a current a color photograph, (passport-style), the authorization number issued after online credit card fee-payment, and a hard-copy and inked FD-258 Federal fingerprint card. (2) A second inked hard-copy FD-258 fingerprint card is also required by the Arizona Department of Public Safety to obtain their Level-One clearance card. Applicant applies at DPS for this licensing requirement. (3) School transcripts can only be accepted directly from the school. National Mblex test results come directly from FSMTB. All out-of-state licenses, prior and current, require that state’s written verification. N OTE: Applicants for an Arizona massage license, from California’s voluntary program, do not qualify selecting "Reciprocity" for their application. (Sample copy of Federal FD-258 form- yellow shows required fields) - Initial license applicants MUST submit 2 FD-258 Federal hard-copy print cards, after fingerprinting; One for State Board, the other for Arizona DPS - NOTE: Initial Arizona massage license applications require the Board online application form, fee payment & attached required materials. Current FBI background check averages 12 weeks. By law, the Arizona Massage Board must wait for Federal national background clearance & DPS Level-One card copy before licensing. To avoid returns, all inked FD-258 Federal fingerprint cards, must have completed fields , including applicant’s valid Social Security number, and legal signature for Federal processing. Avoid Lapse in License & Missed WorkRenew Early Apply when within 60 days from expiration. #1 Complete the online license-renewal application form, attaching the required proof of citizenship/visa, CE certificates, submit online license fee payment. #2 Obtain and attach current/or new copy of your DPS Level-One fingerprint clearance card. #3 Submit online at https://massagetherapy.az.gov/ for processing. Attach copies of CE certificates, Total of 12 CE hours may be online; but the remaining 12 hours are State-required to be in the classroom, physically, with the instructor. Since 2003, Arizona law requires completing 24 Continuing Education hours before submitting a license renewal application. NEED CE HOURS? Arizona LMT’s attending any monthly State Board meeting (virtually), can receive 3 CE hours ( online only ), that will be entered into your licensing record; once each calendar year. One condition for the issuance of an Arizona license is U.S. citizenship, or for individuals visiting the United States legally during that same licensing period.
