Applicant provides a current a color photograph, (passport-style), the authorization number issued after online credit card fee-payment, and a hard-copy and inked FD-258 Federal fingerprint card. (2) A second inked hard-copy FD-258 fingerprint card is also required by the Arizona Department of Public Safety to obtain their Level-One clearance card. Applicant applies at DPS for this licensing requirement. (3) School transcripts can only be accepted directly from the school. National Mblex test results come directly from FSMTB. All out-of-state licenses, prior and current, require that state’s written verification. N OTE: Applicants for an Arizona massage license, from California’s voluntary program, do not qualify selecting "Reciprocity" for their application. (Sample copy of Federal FD-258 form- yellow shows required fields) - Initial license applicants MUST submit 2 FD-258 Federal hard-copy print cards, after fingerprinting; One for State Board, the other for Arizona DPS - NOTE: Initial Arizona massage license applications require the Board online application form, fee payment & attached required materials. Current FBI background check averages 12 weeks. By law, the Arizona Massage Board must wait for Federal national background clearance & DPS Level-One card copy before licensing. To avoid returns, all inked FD-258 Federal fingerprint cards, must have completed fields , including applicant’s valid Social Security number, and legal signature for Federal processing.
